ImpACT+ for primary care staff
We are a multi-disciplinary team (MDT) made up of respiratory specialist nurses, physiotherapists, occupational therapists and consultants working at place level across Southern Derbyshire and Erewash.
We aim to reduce the future burden of respiratory disease to objectively support diagnosis, educate, and respond to the holistic needs to people at risk of or with lung conditions across Southern Derbyshire.

Consultation will be written onto systmOne or handwritten for practices on EMIS.
Please note that ImpACT nurses are not all prescribers.
