If you think you may have symptoms of Coronavirus, follow the national guidance and self-isolate for 10 days. Prescribing concerns

The Prescribing Concerns Portal allows primary care clinicians to highlight patient specific issues that have occurred with prescribing and/or the supply of medicines at discharge or following a clinic appointment at the University Hospitals of Derby and Burton. The information provided will be used to investigate concerns and will help to improve the quality of discharge information related to medicines.

Important: Please contact the discharging ward/clinic directly if an urgent response is required.

Instructions for Using the Prescribing Concerns Form

  1. Report prescribing concerns using the following form by clicking here.
  2. Complete all mandatory information marked with the * symbol.
  3. The ‘prescribing concerns’ email address will appear on the form once all mandatory information has been completed.
  4. Send the form and any supporting documents using a secure email account e.g. NHSmail.

Please note; prescribing concerns related to discharge from the Radbourne Unit should be sent to Derbyshire Healthcare Foundation Trust’s pharmacy team
