Get ready for your surgery by filling in your assessment online
Patients who are on a waiting list for a surgical procedure at University Hospitals of Derby and Burton will now receive a text message, inviting them to complete an online questionnaire called MyPreOp+.
MyPreOP+ allows clinical teams to safely plan patient care, improving patient experience by reducing unnecessary appointments, wait times and related costs.
Patients will be contacted by MyPreOp+ after their initial outpatient appointment and asked to complete the assessment at home prior to surgery. The information is then used to plan the appropriate level of care required, based on individual needs.
Look out for a text message or email asking you to complete the assessment form:
- emails will come from no-reply@ultramed.co
- text messages will be sent by 'Ultramed'
Your email or text message includes a 12 character code, which you’ll use to sign in to MyPreOp with the link provided.
Please complete the assessment form as soon as you are able so that we can begin to plan your care.
The questionnaire will take about 30 minutes to an hour to complete. It might take longer, depending on your medical history.
What happens next
Once completed, the healthcare information you enter is sent directly to the hospital team and someone from your clinical team will call you to talk to talk through the next steps.
What happens if you don’t complete the online assessment
If you don't complete the online assessment, Ultramed will send daily text reminders. After the third reminder, a pre-operative team member will contact you to discuss next steps or alternative arrangements if needed.
What to do if you don’t have internet access or cannot complete the online form
MyPreOp+ saves progress automatically, so you can complete the form over multiple sessions. If needed, ask a trusted person for help completing the questionnaire. If you lack internet access or can't fill out the form, the pre-op team will contact you 3 days after receiving the initial text message to assist or make other arrangements.
