Our Patient Safety Incident Response Plan and what it means for you

In October 2023, University Hospitals of Derby and Burton (UHDB) went live with the latest version of our Patient Safety Incident Response Plan (PSIRP) following the national roll-out of the Patient Safety Incident Response Framework (PSIRF).

PSIRF sets out the way in which patient safety incidents, such as falls, pressure ulcers and healthcare associated infections, are investigated - with an increased emphasis on engaging with our patients and communities, as well as an increased focus on the way in which NHS organisations learn from incidents and improve patient safety as a result.

What does this mean for me?

If you or a member of your family is involved in an incident, the report you receive from UHDB will now follow different templates tailored to the specific incident we are investigating and how this was approached by our teams, which is a new addition in response to implementing PSIRF across the Trust.

We will also look to involve you in more areas of our response to incidents, seeking your views and feedback and answering your questions, making it easier for patients and relatives to have greater transparency into our handling of incidents.

This will help us to make meaningful changes to our practice and improve safety and processes for our future patients.

How will I know if I am involved in a patient safety investigation?

If you or one of your loved ones is involved in a patient safety investigation, you will be contacted directly by one of our teams who will advise you on the next steps of the process and how we will involve you in the investigation itself.
