This page provides public access to our prescribing formulary, and a link to the Joint Area Prescribing formulary for Derbyshire CCGs. These include medicines that the Trust Drugs and Therapeutics Committee consider to have good evidence for safety, effectiveness and value.

Our Formulary incorporates a list of medicines which can be prescribed when treatment is initiated for hospital patients under national and local guidelines and policies.

The Formulary is reviewed and updated monthly and is becoming increasingly evidence based.  NICE Guidance which relates to medicines is processed and incorporated in to the Formulary.  We work closely with directorate pharmacists, medical staff and with the local Joint Area Prescribing Committee (JAPC) on Formulary choices and guideline development.
