We are a multi-disciplinary team (MDT) made up of respiratory specialist nurses, physiotherapists, occupational therapists and consultants working at place level across Southern Derbyshire.
We aim to reduce the future burden of respiratory disease to objectively support diagnosis, educate, and respond to the holistic needs to people at risk of or with lung conditions.
The ImpACT+ service for respiratory patients has made a number of changes to its operation at London Road Community Hospital in Derby in light of the COVID-19 pandemic.
The service is now offering a ‘Virtual Ward’ model, which will facilitate quicker and more supported discharges for patients admitted with respiratory symptoms. Patients need to be safe for discharge (ie able to cope at home), alert with no confusion, and have access to a telephone. Home oxygen and nebulisers can be given if needed.
Suitable patients groups include:
Potential patients are assessed on the ward and discussed with a Respiratory Consultant before being accepted. Once discharged, patients are assessed with a combination of home visits and phone calls for up to a two-week period.
